For Holistic Lifestyle Consultation, Herbology, or Homeopathic Consultation, please fill out all the following forms: Client File Form, Contract of Authorization Form, and Health Consultant Form.

If you have been injured in a car accident and would like us to bill your auto insurance directly, please fill out the Client File Form, the Client Insurance Information Form, and the Signature On File Form. You are also required to have a proper Referral Letter for Massage Therapy from your Doctor.
